I hereby release and forever discharge the West side Flag Football League, their affiliates, managers, members, agents, volunteers, representatives from any physical, psychological injury that I may suffer in any WFFL's games, events, or tournaments.

I grant permission to the West side Flag Football League and its affiliates to use any photographs/videos during any and all league events for any legal use, including and not limited to: publicity, copyright purposes, advertising, and web content. I also understand that no royalty, fee or other compensation shall become payable to me by reason of such use.
